*Bridge Program*
*SUMMARY:* Responsible for planning instruction and/or adaptation of the educational program to ensure that students benefit from a free and appropriate education program in the least restrictive environment for students receiving specialized instruction in a post-secondary transition and community integration program, ages 17-21.
*ESSENTIAL DUTIES AND RESPONSIBILITIES*.

* Instruct individuals and groups in special education skill areas.
* Plans curriculum and lessons using research-based programs and teaching methods.
* Evaluates student achievement based on progress monitoring techniques and observation.
* Collaborates with students, parents, and home school district to develop and implement opportunities to ensure post-secondary transition goals/Indicator 13 requirements are met.
* Collaborate and consult with community agencies and collaborative partners to build academic, pro-social, and adaptive skill opportunities for students.
* Implements behavior support plans for individual students, when needed.
* Monitors students' adaptive/behavioral progress when appropriate
* Maintains physical environment and positive climate of the classroom.
* Maintains contact with parents of students.
* Adheres to the home school and district’s procedures for safety and discipline.
* Prepares report cards, educational records, IEP forms, classroom observation records, and classroom attendance data.
* Maintain professional relationships with colleagues, students, parents, and communities.
* Coordinate and/or participate in individualized education plan team meetings, parent conferences, and other transition community meetings.
* Design and develop transition-related activities for students.
* Coordinate program staff and volunteers to assist with student mentorship, job coaching, work study/internship, and student engagement opportunities.
*EDUCATION and/or EXPERIENCE:*
Bachelor’s degree in education (required); Master’s degree in special education or related field (preferred); Professional experiences with 7-12 special education, post-secondary transition-related services, and community and employment agencies (required).
*CERTIFICATES, LICENSES, REGISTRATIONS:*
Pennsylvania (Inst.
I or II) teaching certificate with special education 7-12 or PK-12 certificate.
QBS-Safety Care certificate(s), or other de-escalation techniques and physical intervention training certification, required by the Intermediate Unit or host agency.
