*General Statement of Responsibilities:* _The Personal Care Attendant is responsible for providing nursing care to residents in the areas of daily hygiene, toileting, feeding, moving safely to activities and other functions and maintaining the cleanliness and care of the resident's' room.
_ *Major Job Duties:* * _Assist residents with daily hygiene to include: bathing/shower; shampooing and combing hair; shaving; oral/teeth-denture cleaning; nail care and applying lotions.
_ * _Assist residents in dressing: dressing for the day; dressing to go out & dressing for bedtime.
_ * _Assist residents with toileting as necessary, including emptying and cleaning bedpans and urinals.
_ * _Assist residents at mealtime to include feeding assistance as necessary and report residents' food intake and output as requested as well as any significant change in residents' eating habits.
Documentation of food and fluid intake and flow record.
_ * _Fill residents' water pitchers and make sure glasses are available witch are cleaned and sanitized.
_ * _Assist residents with: getting in and out of bed or chairs as necessary; in accessing various areas of the facility; getting to meals, activities and other functions.
Use a wheelchair when necessary.
_ * _Assist in the care and handling of residents' laundry and ensure that residents' laundry is properly folded and put away in their room.
_ * _Assist in the cleanliness of the residents' rooms: wipe and clean over bed tables; change linens on beds; clean and organize residents' drawers and closets and ensure they have sufficient supplies.
_ * _Respond to residents' call lights promptly.
_ * _Immediately report any resident accident/incident, noticeable change in condition to the Wellness Director or Administrator.
_ * _Assist in the straightening and the cleanliness and neatness of the linen storage area.
_ * _Project a friendly, helpful, caring and enthusiastic attitude to residents and their families.
_ * _Follow all resident care plans and approaches.
_ * _Ensure documentation is complete and accurate prior to end of shift.
_ * _Perform other duties as assigned by the Wellness Director or Administrator related to the resident care or overall maintenance of the facility operations.
_ *Knowledge, Skills & Abilities in:* _General Personal Care Attendant/Med Tech practices and techniques such as: _ * _Toileting residents_ * _Bathing/Showering residents_ * _Proper and safe lifting techniques_ * _General behaviors and traits of geriatric and how to work effectively with them_ * _Ability to read and write_ * _Ability to show interest and care to residents and their families_ * _Knowledge of: sanitation; universal precautions and safety_ * _Ability to work varied schedules to include weekend shifts_ * _Be prompt and consistent in maintaining work schedule_ *Physical Requirements:* _Must be able to physically assist residents with:_ * _Toileting_ * _Bathing/Showering_ * _Walking_ * _Getting in and out of wheelchairs, beds, and chairs, etc.

_ * _Walking and mobility_ _All applicants must be able to:_ * _Physically stand, move and walk about the facility for an 8 hour shift_ * _Bend and stoop_ * _Exert 50-100 lbs of force occasionally (less than 1/3 of the time) and/or excess of 25-50 lbs of force frequently and up to 10-25 lbs consistently.
_ * _Move objects, equipment and residents.
_ * _Regularly standing, walking, sitting, stooping, kneeling, bending and crouching.
_ * _Using hands to manipulate tools, equipment or controls_ * _Reach with hands and arms.
_ * _Balance_ * _Lift and perform medium to maximum transfer assists with elderly residents.
_ * _Have visual and hearing acuity sufficient to assess residents' safety and ability.
